oxidative stress and homocysteine metabolism following coronary artery bypass grafting by on-pump and off-pump techniques

چکیده
background: it is well documented that coronary artery bypass grafting (cabg) with cardiopulmonary bypass (cpb) causes ischemia and oxidative stress of the whole body. to compare the effect of on – pump and off –pump cabg on the induction of the oxidative stress and the metabolism of homocysteine which is involved in the synthesis of glutathione was investigated in the cabg patients during the early postoperative period. methods: plasma homocysteine, folate total antioxidant capacity (tac) and malonedialdehyde (mda) were determined by standard methods on blood samples obtained from 40 patients undergoing cabg , preoperatively and at 0,12,48,120 hours and 6 months after surgery ,the patients were divided into two matched groups. one of the groups underwent off - pump and the other on – pump cabg. results: a marked reduction of homocysteine, folate and significant elevation of mda were noticed at 0, 12, 48 hours after operation in the both groups (p<0.05). a negative and marked correlation between homocysteine and tac but a positive and significant correlation between homocysteine and mda were observed (p<0.05 in the both groups). in cabg operation because of oxidative stress and consumption of gsh (reduced glutathione) immediate reduction in the plasma levels of homocyteine occurs in the both techniques. however using off pump cabg induction of oxidative stress and changes in plasma levels of homocysteine are not as high as on- pump cabg. conclusion: the on-pump technique was correlated with a faster decrease in the homocysteine level during the first 12 hours and with a faster and higher elevation of the homocysteine concentration 12-48 hours postoperatively.

Effects of off-pump versus on-pump coronary artery bypass grafting: apoptosis, inflammation, and oxidative stress.
BACKGROUND It has been suggested that off-pump coronary artery bypass grafting (CABG) surgery reduces myocardial ischemia-reperfusion injury, postoperative systemic inflammatory response, and oxidative stress. The aim of this study was to measure serum malondialdehyde (MDA), high-sensitivity C-reactive protein (hs-CRP), M30, and M65 levels and to investigate the relationship between M30 levels ...

Surgery for coronary artery disease is known as coronary artery bypass grafting (CABG). It was one of the landmark operations in the history of cardiac surgery that rescued millions of people afflicted by coronary artery disease. It was first performed by Kolesov,1 was popularized by Favaloro,2 and is still the leading heart operation performed today. Current reasons for performing CABG are the...

BACKGROUND Gastrointestinal (GI) complications following coronary artery bypass grafting (CABG), although infrequent, are associated with significant morbidity and mortality. It has been suggested that systemic inflammatory response plays an important role in these complications. Cardiopulmonary bypass (CPB) is well known to cause increased systemic inflammation, and therefore it has been propo...

Coronary artery bypass grafting: on-pump or off-pump? With a growing evidence base surrounding off-pump cardiac surgery, Confluence spoke to Professor Massimo G Lemma, Luigi Sacco University General Hospital, Milan, Italy about how the results of his most recent study investigating on-pump versus off-pump coronary artery bypass surgery in high-risk patients might impact clinical practice. m g L...

Cardiovascular disease is the leading cause of death in the United States and most developed countries. The majority of deaths are due to coronary artery disease. It is estimated that 419,000 coronary artery bypass grafting (CABG) procedures were performed in the United States in 2009 for the surgical treatment of coronary artery disease. Off-pump CABG (without the use of cardiopulmonary bypass...

Evaluating Off-Pump Coronary Artery Bypass Grafting To the Editor: We read with interest the article by Sedrakyan et al.1 The authors found that off-pump surgery was associated with a reduced risk of stroke, atrial fibrillation and surgical wound infections as compared with on-pump. If the results were presented as events, the reductions were translated into 10 fewer patients with stroke, 80 wi...
